How can clients without a legal representative file a lawsuit or ask for an exemption of procedural costs from the 1st of January, 2018? Where is the statement of cost form available?

The new Act CXXX of 2016 on civil procedure (hereinafter referred to as Cp.) and the Act on Administrative Litigation (hereinafter referred to as Ap.) take effect on the 1st of January, 2018.
 
Statement of claim form
According to the new Cp., clients without legal representatives have to use specified forms to submit their statements of claim, written answers to the statement of claim, statements of counterclaim, documents of set-off or other documents containing claims.
 
The Ap. offers the possibility for clients without legal representatives to submit their statement of claim on forms in administrative lawsuits or cases involving administrative omission. 
 
The forms can be found in the 21/2017. (XII.22.) decree of the Ministry of Justice. The National Office for the Judiciary published the forms defined by the Decree in a downloadable format at http://birosag.hu/nyomtatvany-urlapok/keresetlevel-nyomtatvanyok.
 
Exemption of procedural costs form
The rules of bearing procedural costs and the ones concerning cost benefits are changing on the 1st of January 2018 along with new Cp.
 
According to the 26/2017. (XII.27.) decree of the Ministry of Justice about the verification of circumstances confirming cost exemption and the right of cost-recording, said circumstances have to be proved on specified forms with appropriate data content defined in the same decree. The National Office for the Judiciary introduced the form according to the Decree and published it in a downloadable format at the link below:

Clients can also obtain the cost exemption forms on paper at the courts' information office for free.
 
Statement of costs form
According to the new Cp., clients with legal representatives can only charge their procedural expenses via submitting a statement of costs specified by law. In connection with these rules the decree 31/2017. (XII.27) of the Ministry of Justice about statement of costs of procedural expenses, taking effect on the 1st of January, 2018 defines the specific rules of the written statement about charging said expenses and the cost record kept by the clients.
 
The statement of costs mentioned above has to be kept on a unified form. This unified form according to the annex of the decree above, is provided by the National Office for the Judiciary, and can be downloaded from the link below:
